Introduction. Above the Clouds There Is Always Sun

Picture a plane taking off on a gloomy, overcast day. Below — solid gray clouds, rain, murk, and it seems there is no sun at all. But the plane breaks through the cloud layer — and up there, above, there is always sun. It never went anywhere. It simply couldn't be seen from below.

Depression is life beneath dense clouds. And I will say the main thing right away, because whether hope survives depends on it: the sun is still there. Your health, your capacity for joy, your living "I" — none of it has disappeared. It is shut behind the cloud layer of the illness, and that is why you neither see nor feel it. Depression is not proof that there is no sun. Depression is the clouds — and they pass.

I will say this at once, and without false modesty, because science is science: I have the right to lead you above the clouds. I am a psychiatrist, M.D., with 40 years of practice, more than 20,000 clinical examinations, and thousands of completed courses of therapy. I have seen thousands of people at the very bottom of depression and burnout — and I have seen them rise back up to the sun. This is not a consoling phrase. It is what I have observed with my own eyes thousands of times.

First — Against False Consolation

And right away — hard honesty, so that you will believe me. I hate the nauseating positivity that gets flung at depressed people: "just think positive," "smile," "pull yourself together," "just want it and it'll pass." That is not help. It is cruelty and ignorance. To shout at a person under the clouds, "Look, the sun's right there, get it together!" is to understand neither the clouds nor the illness. Depression is often a biological condition, a malfunction in the brain's workings, and you no more cure it with "positive thinking" than you cure a fracture with it. So there will be no magic in this book. There will be the truth about the illness — and a real, honest path upward.

The Foundation of Mental Hygiene, and an Honest Pact

Part I. The Truth About Depression and Burnout

Chapter 1. Sadness, Sorrow, and Depression: Feel the Difference

I will begin with a distinction, without which you can neither understand yourself nor help another. Sadness is not depression. And to confuse them is dangerous in both directions.

Let us sort it out honestly. Sadness is a healthy, living emotion, a response to a specific loss or pain. It has a cause, it has an object, and it passes when the pain burns down; within sadness the capacity for flashes of joy, for contact, for life is preserved. Sadness is not an illness; it does not need to be "treated," it needs to be lived through. Sorrow is a state deeper and heavier, oppressive, often without an object — but still within the range of the living. And depression is no longer an emotion, and not "strong sadness." It is an illness, a persistent state in which the very capacity to feel, to rejoice, to want, to act, breaks down. The key difference: in depression it is not only joy that goes out — the very capacity to experience it goes out (doctors call this anhedonia). The person is not "sad" — he cannot taste anything at all, as if the light inside had been switched off.
